DNV GL – Incident Investigation

In shipping every effort should be made to avoid incidents. This effort, inevitably, requires for us to learn the real reasons why incidents occur and how we can prevent them from happening again. One of the causes for most of the incidents is that previous incidents were not sufficiently analyzed, resulting in symptom fighting alone. That is why the incident investigation process has such a vital role in the overall improvement process of a mature safety management system. Apart from this, OCIMF recommends formal training in Incident investigation and analysis. This course/workshop offers a practical approach towards Incident Investigation and Analysis in the marine environment.
Aim:
This practical course (workshop) aims to provide the tools for proper incident investigation. With examples ranging from “near misses” to “catastrophic events”, we aim to examine how information useful for the improvement can be utilised in a systematic and practicable way.
Participants will be encouraged to share their own experience and have hands-on practice on sample cases.
